Acceptable forms of evidence include registration certificates, licensing agreements, power of attorney documents, signed letters on organization letterhead, or any other relevant document or statement that demonstrates your ownership of the copyrights or your authorization by the copyright holder. If you do not possess these documents, you may provide a signed statement that describes your relationship with the copyright holder.

You can transmit a maximum of 5 files to provide information regarding your request. Please ensure that you do not share any confidential documents or sensitive personal information.
